Résumé

Les analyses effectuées dans nos laboratoires au moyen de diverses techniques analytiques (GCMS, HPLC-DAD,CE-MD) ont montré qu'on pouvait grouper les comprimés analysés en 4 catégories bien distinctes contenant comme principales substances actives:
a) du MDMA ou une molécule apparentée (MDEA, MDA, MBDB, 2C-B);
b) de l'amphétamine;
c) d'autres stimulants (caféine,pseudoephédrine)
d) des substances non apparentées aux stimulants classiques (nicotinate de xantinol, LSD, testostérone, chloroquine...). Les comprimés contenant des mélanges de substances sont fréquents (par ex. amphétamine, caféine, testostérone). A cette variabilité dans la composition s'ajoute une variation dans la teneur en substance active qui peut passer du simple au décuple (par ex. de 9 à 117 mg de MDMA/comprimé (pour 48 comprimés analysés) ou de 8 à 133 mg/comprimé pour l'amphétamine (n = 15)). Des comprimés vendus comme étant de l'ecstasy contenaient chacun une quantité d'amphétamine (133 mg d'amphétamine + 26 mg deMDEA) proche de la dose létale minimale (200 mg pour l'amphétamine). L'analyse chirale de quelques comprimés d'ecstasy de différentes provenances n'a pas révélé de différence significative dans les proportions des énantiomères du MDMA, il s'avère donc que chacun des comprimés analysés possède une composition racémique.Les analyses d'urine montrent souvent la présence supplémentaire de cannabinoides, de cocaine et/ou d'opiacés suggérant que la polytoxicomanie pourrait avoir une certaine ampleur chez les "ravers", amateurs d'ecstasy Les circonstances de demande d'analyse d'échantillons biologiques sont celles de contrôles de circulation routière, de suivi de toxicomanes ou des cas de décès attribués à des intoxications combinées impliquant entre autres une prise d'ecstasy

Methylenedioxy-N-methylamphetamine (MDMA, "Ecstasy') and other related phenylethylamines are nowadays used extensively in western Switzerland at dance clubs and raves .MDMA and related compounds display unique psychoactive properties, acting as a stimulant and inducing feelings of empathy There is a widely held belief among teenagers and misusers that ecstasy is safe. In the last years however an increasing number of reports of MDMA related deaths has been reported. Acute clinical toxicity problems following MDMA ingestion include hyperthermia, convulsion and arrhythmias. There is also growing concern that these phenylethylamines are neurotoxic and cause long-terme damage to serotonin nerve terminals in animal brain.Qualitative analyses by GC-MS of street samples of ecstasy showed that only a part of them contain MDMA or related phenylethylamines (MDA, MDEA, MBDB and 2C-B). Most of them were mixed with caffeine and an excipient (sugars orpolyols (e.g. mannitol)). Amphetamine cut with caffeine and other drugs (e.g. testosterone), stimulants (e.g.pseudoophedrine) or other drugs unrelated to stimulants and phenylethylamines (e.g. LSD, chloroquine, vasodilators) were also detected. Chiral analyses performed by capillary electrophoresis indicated that ecstasy tablets contain racemic mixtures of MDMA. Quantitative determinations by HPLC-DAD reveal large fluctuations in the amount(s) of active(s) substance(s) per tables (e.g. from 9 up to 117 mg MDMA/tablet). Amphetamine amounts (133 mg amphetamine + 26 mg MDEA) close to the minimum letal dose (200 mg amphetamine) were found in some ecstasy tablets. Forensic investigations performed at our institute showed significant blood levels of MDMA, MDEA and MDA in samples drawn from people suspected of driving under the influence of psychoactive drugs. Up to now, no more than one death could be attributed to MDMA intoxication this because our analyses always revealed the additional presence of toxic amounts of other psychoactive drugs (e.opiates, cocaine). Our study shows that because of the variable composition of ecstasy tablets, unpredictable types and amounts of drugs may be taken by ecstasy misusers. Moreover there is considerable concern that trafffic accidents may be caused by MDMA-abusers. MDMA intake could result in severe intoxication and even death especially when combined with other types of drugs.
