RESUME

Au cours des dernières années plusieurs équipes françaises ont attiré l'attention des autorités sanitaires sur la toxicité du buflomédil. Il est admis qu'une ingestion de 3 g (soit 10 comprimés à 300 mg), c'est-à-dire seulement 5 fois la posologie maximale recommandée, peut engager le pronostic vital dans un délai d'une heure. Dans les 17 cas d'intoxications mortelles décrites, les concentrations sanguines post mortem sont habituellement supérieures à 30 mgIl. Nous rapportons trois nouveaux cas, deux potentiellement mortels, avec des concentrations sanguines de 16,8 et 27 mg/1, et un troisième cas mortel avec une concentration sanguine de 14 mg/1.
Ces trois observations alimentent la liste déjà longue des intoxications décrites, surtout en France. Elles mettent en lumière que l'une des complications la plus redoutée - l'arrêt cardiaque - peut survenir pour des concentrations sanguines plus faibles que celles déjà publiées. Il convient donc d'attirer l'attention en particulier des analystes en cas d'intoxication par le buflomédil, la recherche de cette molécule devant être incluse dans le dépistage toxicologique systématique.

SUMMARY

During the past years, several French groups have informed the national health office on the toxicity of buflomedil, as it is well-known that 3 g (equivalent to 10 tablets at 300 mg), corresponding to 5 times the higher dosage, can be lethal within 1 hour From the 17 cases in the literature  fatal blood concentrations were generally higher than 30 mgll.
In this paper the authors describe 2 potentially fatal cases, with blood concentrations of 16.8 and 27. 0  mgll and 1 fatal case, with a blood concentration of 14.0 mgIl.
These findings can be included in the large list of poisoning involving buflomedil, that are frequently observed in France. They document that the major problem noted in case of buflomedil overdose - cardiac arrest - can be observed with low blood concentrations. Therefore, care should be taken when poisoning is achieved with buflomedil, particularly for analysts, that should include the drug in their general unknown screening.
